Seniors' Community Connector - Social Prescribing Program - Mission

Provided by Mission Community Services Society

Provides information and referral to senior citizens (55+) and their service providers.
MCSS Social Prescription program aims to connect seniors who face multiple barriers to accessing community programs, services and/or events by providing resources that aid in improving their wellness and lifestyle. Seniors accessing this program are provided the resources and support that they need to remain connected with their community and live independently for as long as possible. Services offered include:
  • Referral to physical activity programs
  • Skills and literacy training
  • Referral to nutrition and food service programs (grocery shopping, Meals on Wheels)
  • Referral to mental health support
  • Medical transportation
  • Affordable housing assistance
  • Referral to caregiver programs, support programs, education and volunteer experiences.


Access to the Social Prescription Program is available by referral of a doctor or nurse practitioner’s office. Please call 604-826-3634 to begin the referral process for the Social Prescription Program.
