Provides navigation and safety support for seniors and their families managing complex neurodegenerative conditions.
The Senior Helpers team provides specialized navigation and safety support for seniors (65+) and family caregivers managing complex neurodegenerative conditions (including Dementia, Alzheimer’s, and Parkinson’s Disease). They serve as a professional resource for clinicians whose patients require more intensive coordination than the public system can traditionally provide. Services include:
  • Neurodegenerative Disease Navigation: Specialized resource mapping and education for patients with Parkinson’s, Alzheimer’s, and various forms of Dementia.
  • Proactive Safety Reviews: In-home environmental assessments designed to mitigate fall risks specifically for those with cognitive or mobility impairments.
  • Gap Analysis & Advocacy: Assisting families in identifying the specific 'care gaps' between health authority allocations and the patient’s actual clinical needs.

Serves Metro Vancouver and Fraser Valley regions. A referral is not required. Individuals interested in accessing these services can do so by sending an email or phone call.
