Assessment and consultative services for adults 65 and over with onset of mental health issues.
Provides the following services:
  • Assessment and consultative services for adults 65 and over with onset of mental health issues, or whose mental health issue is being complicated by aging, or who is experiencing dementia
  • Consultation to outlying areas regarding case management and treatment planning
  • Interprofessional teams with nursing, clinicians, Psychiatry and GP’s on sessions
  • Education to community, acute services, families and other professions on servicing the elderly population


  • People eligible to access this program would include patients experiencing:
  • Dementia
  • Substance abuse/misuse problems
  • A sudden onset of mental or physical deterioration of an unexplained origin
  • Multiple health or mental health problems that require careful investigation
  • Behaviours or needs that exceed the abilities of the primary caregivers or care facility
  • Mental health issues that have been complicated by age
  • 65 years old and over
