Seniors' Community Connector - Social Prescribing Program - Abbotsford

Program supports seniors to access non-clinical, community-based services to prevent or delay frailty by fostering resilience and social support using a holistic approach.
Social Prescribing is a non-medical, non-clinical referral with the purpose of meeting the need for social and community supports. Seniors’ Community Connector (SCC) works with patient to create wellness plan and continues to follow up as needed.
The Social Prescribing program improves seniors' outcomes by offering more choice and control over their lives and improving a sense of belonging in the community. The program targets root causes of health inequalities and is an important facet of the community-centred practice.
Who Qualifies? Seniors in the Abbotsford area, who may be:
  • Socially vulnerable
  • Emotionally vulnerable
  • Physically frail
  • Regularly attending the ER
  • Physically inactive
  • Experiencing overall poor health outcomes associated with social determinants of health, such as low income

Referrals are made by Health Professionals after evaluating the needs of clients that may benefit from the social prescribing program. Clients may be referred to non-clinical support services within Archway or elsewhere in the community, including:
  • Physical activity programs such as walking groups
  • Skills training
  • Nutrition/food programs, including Lunch With the Bunch or Meals on Wheels
  • Social programs and services, including coffee clubs and community centres
  • Mental health services
  • Medical transportation
  • Affordable housing and Better at Home programs
  • Caregiver programs, support groups, educations sessions, volunteering opportunities and more
