Assertive Community Treatment (ACT)

Assertive Community Treatment teams provide person-centered, recovery-oriented outreach mental health services.
ACT Teams provides intensive support services for individuals with serious mental illness who have very complex needs, and who find it difficult to engage with other mental health services. The service delivery model is client centred, recovery-oriented and provided by a group of interprofessional mental health staff with a typical client to staff ratio of 10:1.

The team is mobile, delivers outreach services in community locations and operates 7 days a week, 12 hours per day.
